1. Type of Change Requested (Specify one of the following: UDC, AAI, Menu, Data Dictionary, Vocabulary Override, or Next Number)

• If UDC, which system code and code type?
• Is a new table needed?
• If AAI, which AAI and Application/Business Process?
• If Menu, which menu? Is a new menu needed?
• If Data Dictionary, which data item?
• If Vocabulary Override, which Application/Form?
• If Next Number, which Company, Doc Type and Business Process?

2. Changes needed - Provide the details of the change(s).

• UDC (system code, code type, value(s), description(s))
• New UDC Table (system code, code type, description for table, code length, 2nd description, numeric, value(s)
• AAI (item number and company, system code and company, business unit, object subsidiary, account use description)
• Menu (selection number, description, menu or job to execute, version)
• Data Dictionary (data item, data type, description, row description, column heading, error message)
• Vocabulary Override (application, form, field, old value, new value)
• Next Number (company, document type, business process, system code, which "bucket", next number value)

3. State Business Reason/Justification

4. Name of Requestor
