If you would like to request an ergonomic evaluation of your workstation:

• Email 4HELP (formerly, Facilities Service Desk) at REWS4HELP/PeopleSoft.
• State in the Subject line that you need an ergonomic evaluation.
• Specify your cube or office location (building and floor)

A NorthStar ticket will then be logged for you and you will be contacted to schedule your evaluation following your request. Do NOT open a NorthStar ticket via the web - it will be cancelled and you will be instructed to send the email specified in this document.

If you have additional ergonomic-related questions, including the replacement of an ergonomic mouse or keyboard that was specially ordered for you, please email Angela Chou. She can also be contacted at (925) 694-2652.
